We want to make your first visit to our office as comfortable as possible. Our office design is relaxing and modern, while our staff is helpful and knowledgeable. As a new patient you can expect only the best from us. To serve you better we have a short list of items to bring on your first visit.
Insurance Card
Our practice accepts most major dental insurance. Contact us to verify
participation in specific insurance plans.
Payment Options
We accept cash, checks, Visa, Mastercard, American Express, and Discover credit cards.
Your estimated patient portion will be due on your scheduled appointment date.
X-Rays
If you have current x-rays, please bring them along on your first visit or please request to have them sent to us from your previous dental office, prior to your first visit.

All new patients must fill out a Patient Information and History form. For your convenience, you can
print this form using the link below. Please print, fill it out, and bring it with you on your first visit to our
office, or allow a few extra minutes before your appointment; we'll be glad to help with them.
